Care Navigator - Hart Health Partnership

We are a large, dynamic and forward thinking GP practice over two sites with 28,000 patients looking for a Care Navigator to join our team (20-25 hours per week).

Job Overview
We are seeking a dedicated and compassionate Care Navigator to join our team. The Care Navigator will play a crucial role in assisting patients and their families in navigating the healthcare system, ensuring they receive the appropriate resources and support. This position requires a strong understanding of office operations, excellent communication skills, and a commitment to providing exceptional care.

Duties

  • Maintain and monitor the practice appointment system
  • Process personal, telephone and e-requests for appointments
  • Answer incoming phone calls, transferring calls or dealing with the callers’ request appropriately
  • Process Healthier Together e-requests as they come in
  • Signpost patients to the correct service
  • Initiating contact with and responding to, requests from patients, team members and external agencies
  • Clinically code data on Emis
  • Data entry of new and temporary registrations and relevant patient information as required
  • Input data into patients’ healthcare records as necessary
  • Direct requests for information, i.e., SAR, insurance/solicitors’ letters and DVLA forms to the administrative team
  • Manage all queries as necessary in an efficient manner
  • Carry out system searches as requested
  • Maintain a clean, tidy, effective working area at all times
  • Monitor and maintain the reception area and notice boards
  • Support all clinical staff with general tasks as requested
  • Action incoming emails and correspondence as necessary
  • As required support Prescription clerks in the management of repeat prescriptions, ensuring they are processed accurately and efficiently
  • Produce, maintain and participate in the receptionist rota for lunch and out of hours cover

We are looking for a candidate to work the following shift pattern:

Tuesday – 07:45 – 13:00

Wednesday – 13:00 – 18:30

Thursday – 13:00 – 18:30

Friday – 13:00 – 18:30

Experience

  • Previous experience in an office environment is desirable but not essential, preferably within a healthcare setting.
  • Strong organisational skills with the ability to manage multiple tasks effectively.
  • Excellent verbal and written communication skills, with a focus on patient-centred care.
  • A compassionate approach to patient care, demonstrating empathy and understanding in all interactions.
