About the role

We are delighted to be recruiting for an Advanced Nurse Practitioner (ANP) to join our friendly and supportive team in this newly created position. This is an exciting opportunity to play an important role in expanding and developing our clinical services whilst improving access to timely patient care.

Key responsibilities:

• Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
• Provide autonomous assessment, diagnosis and treatment for patients presenting with range of acute and chronic medical conditions.
• Independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.
• Implement and evaluate individual specialised treatment plans for chronic disease patients.
• Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patient’s health.
• Support and participate in shared learning within the organisation.
• Work closely with the GPs and Nursing team.

This is a full-time position, and salary is negotiable depending on experience.

About you

We are looking for an experienced and enthusiastic ANP who has:

• Advanced Nurse Practitioner and independent prescribing qualifications
• Experience of working within primary care/general practice
• Experience of assessing and managing minor illness
• Excellent clinical, communication and organisational skills
• The ability to work independently while recognising when referral to a GP or other service is appropriate
• A collaborative and supportive approach to working as part of a multidisciplinary team
