Location: Lordshill Health Centre, Southampton SO16 8HY
Hours: 25 hours a week
Salary: Competitive, dependent on experience
Lordshill Health Centre is a well‑established, busy GP practice serving our local community. We are looking to recruit an experienced Practice Nurse to join our supportive and forward‑thinking nursing team.
⚠️ IMPORTANT:
Applicants MUST have previous experience working as a Practice Nurse within a GP surgery and MUST have experience in chronic disease management.
Applications that do not meet this essential requirement will not be shortlisted.
The Role
The successful candidate will play a key role in delivering high‑quality, evidence‑based nursing care, with a strong focus on long‑term condition management. You will work closely with GPs, fellow nurses and the wider multidisciplinary team to support our patients and help improve health outcomes.
Key Responsibilities
- Chronic disease management, including:
- Diabetes
- Asthma and COPD
- Hypertension
- Cardiovascular disease
- Long‑term condition reviews and care planning
- Health promotion and lifestyle advice
- Cervical cytology (if trained)
- Immunisations and vaccinations
- Wound care and treatment room duties
- Accurate clinical record keeping using the practice clinical system
- Working to QOF and local enhanced service requirements
Essential Criteria
- Registered Nurse with current NMC registration
- Proven experience working as a Practice Nurse in a GP surgery
- Demonstrable experience in chronic disease management
- Ability to work autonomously and as part of a multidisciplinary team
- Strong communication, organisational and clinical skills
- Confident use of clinical IT systems in primary care
Desirable Criteria
- Cervical cytology qualification
- Experience of QOF and long‑term condition templates
