Practice Nurse - Shepherds Spring Medical centre
We are seeking a dedicated and proactice Asthma & COPD Nurse to join our supportive team at Shepherds Spring Medical Centre, situated in Andover with excellent links to Winchester and Salisbury. Hours for the post is negotiable – approximately between 25 to 35 hours per week
We are a busy and well-organised practice serving around 13,300 patients, supported by a multi-disciplinary team including GPs, Practice Nurses, Nurse associate, Clinical Pharmacist, First Contact Physios.
Main duties of the job
- General nursing duties with a focus on asthma and COPD management
- Reviewing and managing patients with respiratory conditions in line with current guidelines
- Supporting QOF targets and helping the practice maintain high-quality outcomes
- Working collaboratively with GPs and the wider team to ensure excellent patient care
Job Role
- To provide excellent patient care
- Treatment room, skills including Chronic disease management in particular asthma and copd management, immunisation, Screening and Health promotion
- Participate in setting standards for care and auditing clinical outcomes.
- Promote & participate in maintaining a safe comfortable & therapeutic environment.
- Participate in developing strategies and implementing health promotion programmes for clients and carers and facilitate screening clinics
- Take responsibility for the safe storage and administration of drugs and equipment
- Participate in setting and monitoring standards to ensure effective care ensuring delivery of evidence-based practice.
- Provide information, advice and support to patients face to face and via the telephone.
- Involved in team discussions to ensure “best practice”
About you
- Be A Registered General Nurse with the Nursing Midwifery Council
- Able to demonstrate evidence of continuing professional development
- Able to work in a changing environment and implement change when applicable to provide high-quality health care
- Be able to demonstrate critical thinking and decision-making skills clinically.
- Be Aware of local and national health policy.
- Must have PC skills, (we use EmisWeb) ideally will be able to use basic word, excel and email
- Highly motivated and flexible.
- Prescribing Qualification would be desirable.
Requirements are:
- Primary Care experience is essential
- To be currently registered with the NMC
- Evidence of continuing professional development
