Wessex LMCs Practice Healthcheck Diagnostic Tool
The Healthcheck Diagnostic Tool has been revised (with thanks to Susie Altmeyer-Ennis for her help), and this now includes PCN roles for practices, and other refinements.
Many practices have found this useful to give a snapshot of where they are now – and may be in 2 & 5 years’ time.

The RCGP endorses the tool and it is used by the Primary Care Development team. NHS England has also published a link to the tool on its own website (with acknowledgment to Wessex LMCs) for use by practices nationally.
