Notification of Practice Changes

Please can practices keep us informed if there are any changes to your practice workforce, in particular to your GPs, Nurses, other health care professionals and Practice Managers as we are no longer receiving updates from the PCSE. Also, please update us if your practice has merged or you are planning to do so, in order that we can keep our database as up to date as possible and ensure relevant information is sent to the correct people.

Therefore, going forward can we kindly ask you to please complete the attached “Notification of Practice Changes” form and email it to the LMC office (office@wessexlmcs.org.uk) with any relevant changes so we can ensure our records are as accurate as possible.
