Practice Nurse - The Porch Surgery
The focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions. Management of diabetes is a key area of this role, other areas, e.g. hypertension, peripheral vascular disease, chronic kidney disease and cerebrovascular disease are desirable. Although long term chronic conditions is the main purpose of the post, the post holder will if they choose have the opportunity to provide a general practice nurse service which could include cytology, wound care, immunisation and other core General Practice Nurse roles, as required and within your competencies.
