Healthcare Support Worker - Hart Health Partnership

Hart Health Partnership is a large Practice in Fleet, Hampshire, serving a list of 28000 patients. We have a diverse multi-disciplinary team with 22 GPs, 11 Nurses, 4 paramedics, 1 ANP as well as other allied colleagues such as First Contact Physiotherapists, Pharmacy colleagues, Mental Health Support Worker and a large care navigation and admin team.

Healthcare Support Workers manage a caseload of patients, often working closely with colleagues in a multidisciplinary team (MDT). This role will support both clinical and administrative teams in the delivery of clinical and administrative services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. This is not a clinical role.

ROLE AND RESPONSIBILITES

The following are the core responsibilities of the Healthcare Support Worker. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

  • Weight management support
  • Pre-diabetes/Gestational Diabetes
  • Co-ordination of Palliative Care Register
  • Cancer Care Reviews – Holistic signposting and support
  • Dementia Reviews
  • Smoking cessation
  • Deliver and assist clinical staff in the provision of treatment, preventative care
  • Health promotion, long-term condition management and patient education
  • Liaise with patients who are struggling due to changes in their health and social situation including patients who are elderly, have dementia, have mental health issues or a learning disability
  • Weekly contact with care homes for proactive support
  • Wellbeing initiatives for the whole HHP team
  • Weekly proactive care meeting attendance inter-working with our Integrated Care Team
  • Building links with the community and voluntary services to support our patients in their own home
  • Chasing patients on our QOF register with outstanding actions
  • Carers champion
  • Support patients to prevent a crisis situation

We are looking for 37.5 hours per week.
