CHD Practice Nurse, North Baddesley Surgery

North Baddesley Surgery is looking for a highly motivated CHD Practice Nurse to join our existing Practice Nursing Team.

We are a training practice based over two sites, North Baddesley Surgery and Knightwood Surgery, with a list size of approximately 10,000 patients. We use EMIS Web, AccuRx and eConsult. Our practice team comprises three GP Partners, two Salaried GPs, one Retainer GP, two ANPs with dedicated and supportive Reception and Administration Teams, led by our Practice Manager and Deputy PM. We are part of Romsey and North Baddesley Primary Care Network, with our patients benefiting from knowledge from First Contact MSK practitioners, Health & Wellbeing Team, Vulnerable Families Team and Pharmacists who work across the PCN.

We are seeking an experienced Practice Nurse who can deliver safe and effective CHD care and reviews to our patients, as part of our Nursing & HCA Team. The team is led by our Nursing Lead, with support from the Management Team and Nursing Administrator.

Hours: 10 hours per week, ideally split as 2x five-hour days, but with flexibility around working days

Salary: Dependent on experience

Person Specification

 Qualifications:

Essential:

  • RGN – current NMC registration
  • Qualification in CHD chronic disease management
  • Meet the requirements and produce evidence for nurse revalidation
  • Evidence of continuing professional development (CPD) commensurate with the role of a Practice Nurse

Experience:

Essential:

  • Experience of working as a practice nurse
  • Experience of chronic disease management
  • Experience of autonomous working

Skills:

Essential:

  • Excellent communication skills (written and oral)
  • Strong IT skills
  • Clear, polite telephone manner
  • Competent in the use of Office and Outlook
  • Effective time management
  • Ability to work as a team member and autonomously
  • Good interpersonal skills
  • Problem solving & analytical skills
  • Ability to follow clinical policy and procedure

Desirable:

  • EMIS user skills – full training can be provided
  • Qualification in Diabetes management
