Clinical Pharmacist - Mayfield Medical Centre

The post holder is a clinical pharmacist, who acts within their professional boundaries, in close co-operation with and fully supported by other members of the clinical team at Mayfield Medical Centre.

At the end of every clinical session at Mayfield we have a designated slot of clinical time for both pharmacists and GPs to discuss patient care and provide mutual support. We have found this particularly important for pharmacists early on in their transition from community/hospital pharmacy to becoming autonomous practitioners in general practice.

The post holder will be an integral part of the practice team, working with others in the PCN, as well as part of a wider pharmacist network and multidisciplinary team. This includes being a conduit of patient medicines information into and out of local hospital and community pharmacy. They will work to optimise medication issues to improve patient care and safety and support clinical staff in the management of patients.

As part of their employment, the post holder will achieve qualifications from an accredited training pathway, including independent prescribing, equipping the clinical pharmacist to be able to practice and prescribe safely and effectively in a primary care setting.

A supervision and mentorship network will be provided to support the clinical pharmacist.


You should be

Experienced in primary care, motivated and have a passion to deliver an outstanding service within general practice. The successful candidate will be a team player with excellent communication and organisational skills, capable of forging productive working relationships within the practice whilst also being able to work alone.


We are

Salus is a GP Federation which was formed by the local GP practices within North East Hampshire and Farnham. This Includes practices located in Yateley, Farnborough, Fleet, Aldershot & Farnham.

We work on behalf of local General Practice to provide services at scale to our member practices.

We have a very successful team of Paramedic Practitioners working to provide home visiting services, on the day clinics in practices and nursing homes.

We work at the centre of Integrated Care locally with our own team of nursing, clinical associate, care- co-ordinating and administrative staff.

We act as an employment vehicle for ARRS staff working in local PCN’s/ Practices.

We were part of the first wave pilot scheme welcoming pharmacists to come and work in general practice back in 2016 and have employed pharmacists ever since.

The senior management team at Salus Medical Services have a wealth of experience working in general practice and therefore a good understanding of the opportunities and challenges faced.

Since 2019 we have held the APMS contract for the running of Mayfield Practice in Farnborough.

Mayfield Medical Centre is a 9000-patient practice operating out of modern premises with a very diverse patient population. We have a strong team of GP’s, Practice Nurses, Paramedic Practitioners, Mental Health Support Workers, MSK Physiotherapists and Care Coordinators.  We work very closely with Farnborough Primary Care Network of 6 GP practices, serving a   population of over 60,000patients.

All 6 practices are fully engaged with the PCN, and each have a GP as a board member.   We strongly encourage our practice pharmacists to meet with, learn from and provide mutual support to their colleagues with Farnborough PCN and more widely within North East Hampshire.
