Care Co-Ordinator - New Horizons Medical Partnership
New Horizons Medical Partnership is a two site practice based in Totton Hampshire.
We are looking for an enthusiastic individual who would like to be part of a team of Care Co-ordinators helping to support our patients in receiving the appropriate care. New Horizons Medical Partnership is a very friendly and supportive environment and we are looking for someone to act as the first point of contact and representative for the practice either by telephone or face to face joining the team on a part time basis for 28.5 hours per week 8am – 6.30pm Tuesday, Thursday & Friday.
The role could be based at either of our two sites both in Totton Hampshire.
Closing date is: Tuesday 9th June 2026 (11.59pm). Interviews are scheduled to be held week commencing Monday 15th June 2026. However, please do not delay applying for this vacancy as some interviews may be held prior to the closing date if suitable applications are received.
Main duties of the job
- Deliver excellent customer service to our practice population
- Work collaboratively with the general practice team to meet the needs of our patients
- Facilitate effective communication between patients, members of the primary health care team, secondary care and other associated agencies.
