Care Co-ordinator – Urgent Care Team/Frailty - Hart Health Partnership
Hart Health Partnership is a large, newly merged practice in Fleet, Hampshire, serving a list of 28000 patients. We are based over two sites in close proximity of each other, within the heart of the Fleet Community. We have a diverse multi-disciplinary team with 21 GPs, 8 Nurses, 8 paramedics as well as other allied colleagues such as First Contact Physiotherapists, a Health & Wellbeing Coach, and Pharmacy colleagues.
We have an opportunity to recruit a Care Co-Ordinator and can offer flexibility of working hours of approximately 37.5 hours per week, job share considered We welcome applications from those with primary and/or secondary care experience and welcome those with additional qualifications too.
The Care Co-ordinator for our Urgent Care Team and Frailty at Hart Health Partnership will have a key role in supporting the Urgent Care Team and GPs to ensure the smooth running of our urgent care system.
Working in our Urgent Care hub with Duty Doctor & our Practitioner team, you will be a key link between the Urgent Care team & a range of in-house and external colleagues, and will support the team with patient care navigation & signposting to services that can address their further care needs.
Tasks include but are not limited to acting as administrative support for the team, carrying out some simple clinical tasks such as blood tests and urinalysis and being a point of contact for vulnerable patients.

The role is full time based at Branksomewood Road Surgery.
This post will be supervised, and line managed by the Urgent Care Team Lead, Practice Managers and Partners.
