Sea City Partnership has an exciting opportunity to join our advanced practice team, providing health care to an undifferentiated caseload of patient presenting with acute and chronic health conditions, within our practice sites or in the patients own home.
Main duties of the job
- To ensure that the needs of the patients are placed at the centre of care delivery
- To deliver a high standard of patient care as an advanced practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base
- To manage a clinical caseload of patients presenting with undifferentiated health needs, responding effectively to patient need and ensuring patient choice and ease of access to services.
- The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers, and colleagues, ensuring clinical safety and excellence.
- Prescribe safe, effective and appropriate medication as defined by current legislative framework
- To undertake and promote confident, safe, and autonomous practice. To engage with continuous professional development and to work with the advanced practitioner and wider practice team to develop competencies to expand the role of advanced practitioners as a core part of the Sea City clinical team.
- To remain clinically competent, registered with appropriate governing body as relevant to profession and ensure continuous professional development opportunities are undertaken to keep up-to-date and then disseminate clinical best practice guidance
- To contribute to the development and delivery of strategic plans for the development of Sea City Partnership services.
The advanced practitioner will be supported in their role by the advanced practitioner lead, GP partners and GPs within the practices. There will be opportunities for upskilling and further training within the role to meet the needs of the service if necessary. Job share and part time applications are also welcome.
Core practices opening hours of 8:00am 6:30pm Monday to Friday each week.
