Advanced Nurse Practitioner - South Coast Medical Group

South Coast Medical Group is an award-winning NHS Primary Care Home, comprising six GP surgery locations in the Bournemouth general area and serving over 50,000 patients in total.

By working at scale, we are better able to promote health and provide for the differing needs of the communities of local people and a balanced workload for our staff.  It also enables us to employ a significant multi-disciplinary team including Pharmacists, Advanced Nurse Practitioners and Paramedics (for pre-assessment screening and home visits) all of whom reduce the clinical workload for GPs.

An exciting opportunity has arisen for an experienced full time Advanced Nurse Practitioner/ Advanced Clinical Practitioner to join our friendly expanding General Practice in Bournemouth Area. You will be part of a dynamic and innovative team who value teamwork, professional practice, mutual support and a friendly working environment. A recent development within the practice is the formation of an Advance Practice Forum to facilitate closer working relationships and peer support amongst the advanced practitioner team.

What are the main duties of the role?

  • You will be responsible for providing initial telephone triaging and assessing the need for face to face appointments.
  • You will act as an autonomous clinician and take responsibility for the assessment, treatment plan and/or to appropriately refer patients to specialist services if required.
  • Be compliant with the NMC or HCPC Code of Professional Conduct and any associated national guidelines and abide by their guidance.
  • Take a comprehensive patient history, carry out physical examinations and use expert knowledge and clinical judgement to identify the potential diagnosis.
  • Refer patients for investigations and/or the appropriate specialist where appropriate.
  • Make a diagnosis.
  • Independently decide on and carry out treatment, including the prescribing of medicines.
  • Make onward referrals to other services as appropriate.
  • Ensure the provision of continuity of care.
  • Work independently, but as part of a local and wider health care team.
  • Provide leadership to the wider team and make sure that each patient’s treatment and care is based on best practice.

What we look for in your application:

  • Registered Advanced Nurse Practitioner with the NMC (Nursing and Midwifery Council) or a qualified paramedic registered with the HCPC (Health and Care Professionals Council) or other relevant body.
  • 2+ years proven record of acting as a registered nurse /paramedic practitioner / urgent care practitioner.
  • As an independent prescriber, you will be responsible for safely prescribing medication to complement treatment plans for a range of medical conditions.
  • Background of working in a fast-paced Primary Health Care or acute environments is essential.
  • Proven background of dealing with a diverse range of patients from all walks of life.
  • Proven background of building positive rapport with a diverse range of internal colleagues and clients.
  • Proven ability to work autonomously whilst also having an awareness when identifying the need to access the wider clinical team or external agencies appropriately and safely.
  • Hold a Full UK Driving Licence.
  • You will have a proven record of acting in roles where you have acted autonomously and responsibly for the assessment, treatment and/or appropriately refer patients to specialist services when required.
  • This role is particularly suited to an autonomous clinician with a proven track record of delivering high quality care within the Primary Care setting with experience in dealing with a wide range of minor ailments.
