JOB SUMMARY
We are seeking a dynamic and experienced Advanced Clinical Practitioner (ACP) to join our dedicated multidisciplinary team. The successful candidate will play a key role in providing high-quality care, working autonomously to assess, diagnose, treat, and manage patients with a wide range of acute and chronic conditions.
KEY RESPONSIBILITIES
· Provide expert clinical care and independently manage a caseload of patients
· Undertake clinical assessments, including history taking, physical examinations, and formulation of treatment plans
· Prescribe medications within scope of practice
· Support chronic disease management
· Deliver health promotion and disease prevention advice
· Work collaboratively with GPs, nurses, and allied health professionals
· Participate in clinical audits and quality improvement initiatives
PERSON SPECIFICATION
Essential:
· Registered health care professional
· Master’s level qualification in Advanced Practice or equivalent experience
· Independent Nurse Prescriber (V300)
· Significant experience in a primary care or urgent care setting
· Excellent communication and interpersonal skills
· Ability to work autonomously and as part of a team
Desirable:
· Experience in managing long-term conditions
· Minor illness and injury qualifications
· Knowledge of SystmOne
