About the Practice

• Established partnership with visible, engaged leadership
• Structured triage model to manage demand safely
• Multidisciplinary team including ANPs, Clinical Pharmacists, Paramedics and experienced Nursing team
• Dedicated administrative and document management support
• Active involvement in PCN services

We actively design our systems to reduce unnecessary GP workload rather than shift it.

Our Clinical Model

We operate a structured triage system that:

• Ensures appropriate clinical prioritisation
• Directs patients to the most suitable clinician first time
• Reduces inappropriate GP appointments
• Supports continuity where clinically indicated

Routine demand is balanced through:

• MDT allocation
• Defined appointment templates
• Clear capacity planning
• Managed home visit allocation

The Role

• Routine GP clinics
• Participation in triage rota (supported model)
• Minimal home visits (supported by paramedic team)
• Shared duty responsibilities
• Opportunity to develop special interests (if desired)
• Participation in Urgent Care Sessions at our hub
