Wessex Healthier Together

The “Wessex Healthier Together” project is exploring how parents, carers and community services might be better supported and integrated with hospital based services so that children are able to access the ‘right care at the right time from the right person’.

The Wessex Healthier Together website is being developed to provide useful resources for parents, carers and young people (via information leaflets, self-care advice, & guidance on when and how to access appropriate local healthcare services). The site also supports healthcare professionals (via standardised local clinical guidelines, educational resources & referral pathways).

Wessex Healthier together has now grown to be provide an offer to parents expecting children alongside it’s more traditional paediatric decision support. There is now an App which parents can download to support pre-partum health decisions alongside the better know paediatric decision support tools. A practice can now on board the Wessex Healthier Together tool kit so than patients can use the App as a decision support tool. Any time the tool suggests they need GP Practice support it can then pass this information direct to the practice, similar to other online consulting tools. By utilising the tool it is hoped patients are more likely to follow the sign posting guidance rather than rely on the traditional “Call the Dr”  default. In turn this will both educate patients on self care alongside managing primary care demand.

Dr Will Howard (Medical Director Wessex LMCs and GP) recorded a podcast with Dr Sanjay Patel (Southampton Hospital Consultant in Paediatric Infectious Diseases) to help Practices further understand how Wessex Healthier Together can be used in General Practice.
