PCN Nurse – Home Visiting & Proactive Care Team (2-4 days per week) - Southampton West PCN Limited
Southamtpon West PCN is looking to recruit an enthusiastic, motivated nurse to join our growing multidisciplinary home visiting team, supporting patients who are unable to attend the surgery.
This is an excellent opportunity to work at scale across our member practices as part of a small, supportive team, focusing on high‑quality, proactive care for some of our most vulnerable patients.
About the role
Key elements of the role will include:
- Undertaking planned and urgent home visits for housebound/care home patients and those unable to attend the surgery
- Assessment and management of acute illness within your scope of practice, with senior clinical support readily available
- Proactive, person‑centred conversations with patients and carers, including anticipatory care and self‑management support
- Contributing to QOF and other quality/improvement work for long‑term conditions in the housebound population
- Working closely with GPs, clinical pharmacists, social prescribers and other ARRS roles within the PCN
About you
We are looking for someone who:
- Is a registered nurse with current NMC registration
- Has experience in primary care, community nursing or a similar setting
- Is comfortable working autonomously in patients’ homes, but as part of a wider multidisciplinary team
- Has good communication skills and a proactive, patient‑centred approach
- Is an independent prescriber, or willing to work towards this (support can be provided)
- Is confident in assessing and managing acute presentations within their competence, with appropriate escalation
A full UK driving licence and access to a car for work is essential.
