Sessional Salaried GP - Clift Surgery
This role will focus on delivering both proactive and reactive care for patients living with frailty within our practice population. Working in close partnership with our practice team and dedicated Frailty Team and NHS system partners, you will help ensure high-quality, personalised care that supports patients through winter pressures and beyond. The postholder will play a key role in:
- Identifying and managing frailty proactively to reduce avoidable admissions and improve patient outcomes.
- Responding reactively to acute needs, ensuring timely, compassionate interventions.
- Collaborating with multidisciplinary colleagues and external partners to deliver integrated, patient-centred care.
- Supporting continuity of care and resilience in service delivery during periods of increased demand.
