Safe Prescribing of Anticoagulation ( Warfarin )
The NPSA issued guidance on the safe prescribing of anticoagulants in 2007
- ensure before issuing any anticoagulant prescription that the patient's INR is being monitored regularly and that it is at a safe level for a prescription to be issued
- Changes to medication that might interact with warfarin must be communicated to the clinic or team monitoring the INR and additional testing ensured
Additionally if you are involved in the monitoring and dosing of anticoagulants
- Patients when initiated on therapy must receive adequate verbal and written information. (Patients should have a "yellow book" which has information, advice, appointments, results and dosages recorded.)
- Ensure any dose changes are IN WRITING to the patient or their carer
- Ensure staff involved in prescribing, monitoring and advising patients are competent and trained*
- You should participate in an annual audit of your service
- You should write, review and update protocols and clinical procedures that relate to anticoagulation for which you or your staff have responsibility
