Employees Statement of Sickness Forms (Self certificate or SC2)

Individuals may make a statement of sickness for the first week of sick leave and GPs are not required to issue certification for periods of less than one week.

You may chose to provide a private certificate for the first week of sick leave if requested to do so. However, we would recommend that you resist this. It is not appropriate use of NHS resources for individual's with self limiting minor illness to attend solely for the purpose of satisfying their employers need for a "doctor's note".

Wessex LMCS has generated a letter that may be issued to employers requesting this of their employees.
