Introduction

The study session will explore the HCA role in wound care, considering expectations, variability in roles and challenges.

The clinical part of the session begins with identifying the stages of wound healing. How to accurately describe the wound bed, including identifying different tissue types and key wound descriptors. The session will explore wound bed preparation including care of the peri-wound and surrounding skin.

The session will discuss the key factors to consider when selecting dressings. We will explore the various types of dressings on your formularies, including how they work, what they do, indications and contraindications.
Basic management of leg ulcers will also be explored including aetiology, dopplers, compression therapies, leg care, challenges and referral (please note this is a theoretical session and there is no practical doppler or compression application in this session).

The session will include various case study examples to embed theory into real world scenarios.

Aims

This session aims to discuss and explore the following:

  • The role of the HCA, including opportunities and challenges
  • The Stages of Wound Healing, how to assess a wound, and dressing selection
  • Wound bed preparation including assessing, cleansing, debriding and biofilm management
  • Care of peri-wound and surrounding skin including use of emollients
  • Care planning and the importance of the continuity of care
  • Basic leg ulcer theory including aetiology, dopplers, compression therapies, leg care and challenges
  • Putting theory into practice with case studies and scenarios
  • Learning Resources
  • When and to who to refer on to

Objectives

This session will support attendees to explore their role in wound care and the key part they play in the care of patients with wounds. The session will give the attendee the theory and key elements to wound assessment and dressing selection. The trainer will provide guidance to support evidence-based understanding of basic leg ulcer management (it is recommended that attendees sign up for one of Wessex LMC’s practical leg ulcer/ doppler sessions before completion of these competencies in their practice). The session will use case examples and scenarios to embed theoretical knowledge.

NB: The session will not deem the learner 'competent'.  The ‘journey to competency’ will be explored using national capability framework resource and subsequent eLearning modules, as will contacting the Tissue Viability team to access any local policies, documents and tools to support their learning that may be available.  Please see this short video 'A guide to becoming competent'.

Matthew currently practices as an Advanced Tissue Viability Nurse Specialist working within a Hampshire NHS Trust.

He spent most of his career working in a community nursing setting, building a passion for wound care. In 2017 he became the first ‘Enhanced Wound Care Nurse’ in his trust, where, using enhanced knowledge and skills provided by the Tissue Viability specialist team, Matthew was able to evidence both better patient outcomes as well as a reduction in the number of wound care patient visits. Being successful in the role has led to the role out of ‘Enhanced Wound Care Nurses’ across the trust.

Matthew has taken the opportunity to become a community team lead on a secondment to hone his leadership and management skills, but was always keen to return to his passion and sought out a position in the local Tissue Viability Specialist Team as Advanced Tissue Viability Specialist Nurse. Matthew is an independent nurse prescriber.

Within his current role Matthew provides support, guidance and training to Primary Care across multiple practices in Hampshire. He regularly provides wound care training for community practitioners, community hospital practitioners, mental health practitioners, primary care practitioners, and nursing home nurses.
