This is a study session for experienced Practice Nurses who are competent in wound care. The session explores realistic and practical solutions to the barriers/dilemmas faced when providing care for patients with complex wounds and those that are deemed ‘hard to heal’. The session will explore factors that make a wound ‘complex’, or ‘hard to heal’. The session will briefly explore the need for full lower limb assessment, doppler and subsequent compression therapy when managing leg ulcers. The session will explore factors that can make a wound complex, including differential diagnosis. For hard to heal wounds, the session will explore some of the common barriers/dilemmas; focussing on practical and realistic solutions towards healing or achieving patient goals of care.

The session will use patient scenarios which will involve delegates in discussion around some of the barriers making the wounds ‘hard to heal’ or ‘complex’ and possible solutions to overcome some of these.

Aims

The session aims to explore and discuss:

  • Defining ‘complex’ and ‘hard to heal’ wounds
  • Exploring the basic need for lower limb assessment, doppler, and compression therapy for leg ulcers
  • Looking at differential diagnoses e.g. Cancer, Pyoderma, vasculitis, medication induced etc
  • Practical and realistic solutions to common reasons that make a wound ‘hard to heal’ or ‘complex’ – e.g. Biofilm, pain, poor nutrition, mobility, and compliance/concordance with treatment
  • A chance for the delegates to discuss some of the common issues/barriers to healing wounds that they face in practice alongside patient scenarios being discussed in the session
  • Next steps

Objectives

Following this session, the learner will gain practical advice and guidance to aid the application of appropriate therapeutic treatments, solutions or signposting, in cases of patients with complex or hard to heal wounds. Please note that this session will not include any competency sign off.  Please see this short video 'A guide to becoming competent'.

Matthew currently practices as an Advanced Tissue Viability Nurse Specialist working within a Hampshire NHS Trust.

He spent most of his career working in a community nursing setting, building a passion for wound care. In 2017 he became the first ‘Enhanced Wound Care Nurse’ in his trust, where, using enhanced knowledge and skills provided by the Tissue Viability specialist team, Matthew was able to evidence both better patient outcomes as well as a reduction in the number of wound care patient visits. Being successful in the role has led to the role out of ‘Enhanced Wound Care Nurses’ across the trust.

Matthew has taken the opportunity to become a community team lead on a secondment to hone his leadership and management skills, but was always keen to return to his passion and sought out a position in the local Tissue Viability Specialist Team as Advanced Tissue Viability Specialist Nurse. Matthew is an independent nurse prescriber.

Within his current role Matthew provides support, guidance and training to Primary Care across multiple practices in Hampshire. He regularly provides wound care training for community practitioners, community hospital practitioners, mental health practitioners, primary care practitioners, and nursing home nurses.
