Introduction
This live-online, instructor led session is designed to support HCAs who are involved in long-term condition reviews.
Aims
The aims of this session are to:
- Support Healthcare Assistants (HCAs) in developing knowledge and confidence when contributing to long-term condition (LTC) reviews.
- Provide a clear understanding of the evidence-based advice HCAs can safely deliver to patients with respiratory conditions, hypertension, and diabetes.
- Ensure HCAs are able to recognise boundaries of practice and know when referral to a clinician is required.
Objectives
By the end of the session, participants will be able to:
- Describe the key elements of long-term condition reviews for respiratory disease, hypertension, and diabetes.
- Deliver evidence-based lifestyle and self-care advice relevant to these conditions.
- Recognise red flags, complex presentations, or abnormal results that require escalation to a nurse or GP.
- Understand their role within the wider multi-disciplinary team in supporting patients with LTCs.
- Develop confidence in knowing when and how to refer patients on for further assessment or treatment.
